The practice and your information
The website’s private appointment service is provided by Ortopeym Limited (company number 13087097), with clinical care provided by Mr Peyman Bakhshayesh. For questions about the handling of your consultation information, contact peyman.bakhshayesh710919@gmail.com or 07427 297799. The practice’s correspondence address is 4 Parklands Avenue, Leeds, LS17 6TB.
What this form collects
We collect your name, date of birth, contact details, current complaint, relevant history, medicines, allergies and information about available imaging. Gender, GP details and appointment details are optional. The form does not accept imaging files. Age is calculated from your date of birth. The clinician may add an assessment, advice and follow-up record after your consultation.
Why we use it
We use this information to prepare and provide the consultation you request and maintain an accurate clinical record. Providing the required information helps the clinician assess whether remote advice is appropriate. The form does not book an appointment or replace an examination where one is needed. You may contact the practice if you need a different way to provide the information.
For requested private clinical care, the relevant grounds are performance of a contract or steps you request before a contract (UK GDPR Article 6(1)(b)), together with provision of healthcare under professional confidentiality (Article 9(2)(h)). Any legally required retention uses the applicable legal-obligation ground; handling legal claims may use legitimate interests and Article 9(2)(f). Optional sharing decisions are discussed with you during care; the form’s accuracy acknowledgement is not blanket consent to every use of your records.
Storage and access
Submissions and saved consultation entries are encrypted in the clinical database. The clinician area requires sign-in and restricts access to the practice’s configured clinician account. This form does not email your medical answers to an inbox, place them in a calendar booking or save them in browser local storage. Technical suppliers, including the Sites hosting service and Cloudflare, process information to operate and secure this service.
This is hosted storage, and no UK-only hosting or end-to-end encryption claim is made. Hosting and support arrangements may involve processing outside the UK. Contact the practice for details of applicable supplier arrangements, locations and transfer safeguards before submitting if you need this information to decide whether to use the online form.
Sharing and copies
Relevant information may be shared with your GP or other treating clinicians as part of your care, following discussion with you. Disclosure may also be required by law or justified to protect someone from serious harm. Information is not published on this website. You can ask for a copy of your information or a consultation letter; the practice will verify your identity and agree an appropriate delivery route.
How long information is kept
Clinical information is kept for the period appropriate to the type of record, your ongoing care and applicable professional, legal and claims requirements. Uncompleted enquiries are kept only while necessary to deal with the request and associated obligations. Contact the practice for the retention period that applies to your record. A request to delete information is considered alongside any requirement to retain clinical records.
Your rights
You may request access, correction, and where applicable erasure, restriction or portability, or object to relevant processing. These rights have exceptions. Corrections to clinical entries are recorded as dated additions so the original clinical record remains traceable. Contact the practice using the details above. You can also raise concerns with the Information Commissioner’s Office.
Urgent concerns
This form is not continuously monitored and must not be used for emergencies or urgent medical advice. In the UK call 999 for an emergency, or NHS 111 for urgent advice.
